- Location : Los Alamos, NM\n
- Setting : Post-acute care, inpatient (typically skilled nursing and rehab facilities)\n
- Schedule : \n
- Preferred full-time : 3.5 5 days / week\n
- Part-time options available : 1 3 days / week\n
- On-call : Daytime call only, Monday through Friday on assigned days\n\nResponsibilities : \n
- Provide comprehensive primary care in a post-acute setting\n
- Manage both acute and chronic medical conditions\n
- Create individualized care plans\n
- Collaborate with an interdisciplinary care team (IDT)\n
- Partner with supervising physician and facility staff\n\nQualifications : \n
- Licensed NP (FNP, AGNP, CNP, APRN) or PA\n
- Active New Mexico license or eligibility\n
- DEA license preferred (not required)\n
- Experience in post-acute care, outpatient, urgent care, primary care, home care, or inpatient settings is welcomed\n
