RN Case Manager Job Details

Estimated Weekly Salary : $1,870–$1,920 per week

Assignment Type : Locum tenens (travel RN contract)

Location : Harvey, IL 60426 (Chicago metro area)

Healthcare Setting : Community hospital

Start Date : October 20, 2025

End Date : January 19, 2026

Contract Duration : 13 weeks

Shift : Day shift, 8 : 00 AM – 4 : 30 PM

Schedule : 5x8-hour shifts per week (40 hours / week)

Patient Population : Adults and geriatrics, daily census 20-25

RN Case Manager Qualifications

Active Registered Nurse (RN) license in Illinois (IL) – required at time of application

Minimum 3 years of recent case management experience in an acute care hospital

BLS certification (American Heart Association) – required

ACM (Accredited Case Manager) or CCM (Certified Case Manager) certification preferred

Proven experience with care coordination, discharge planning, and HIPAA compliance

COVID-19 vaccination required (medical / religious exemptions accepted)

Two professional references, including one from a direct supervisor or nurse manager

RN Case Manager Responsibilities

Coordinate patient care and discharge planning for adult and geriatric patients

Collaborate with interdisciplinary healthcare teams, including social services, to achieve optimal patient outcomes

Manage a daily caseload of 20-25 patients, delivering comprehensive case management services

Ensure compliance with HIPAA regulations and hospital policies

Document and communicate individualized care plans with patients, families, and healthcare professionals
