MDS Coordinator

Pinnacle Care , a leading nursing home facility in Battle Creek, MI, is seeking a highly motivated and experienced MDS Coordinator to join our team. As the MDS Coordinator, you will be responsible for ensuring the accuracy and completeness of MDS assessments and care plans for all residents in the facility.

Key Responsibilities :

Conduct and coordinate the completion of MDS assessments in accordance with state and federal regulations.

Ensure accuracy and completeness of MDS assessments by reviewing medical records and consulting with interdisciplinary team members.

Develop individualized care plans based on MDS assessments and resident needs.

Maintain accurate and complete records of MDS assessments and care plans.

Monitor and report on resident care and progress to ensure quality care is being provided.

Collaborate with interdisciplinary team members to ensure timely and appropriate interventions for residents.

Qualifications :

Valid RN license in the state of Michigan.

Minimum of 2 years of experience in MDS coordination in a nursing home setting.

Thorough understanding of state and federal regulations related to MDS assessments and care planning.

Excellent organizational, communication, and interpersonal skills.

Ability to work effectively as part of an interdisciplinary team.

Strong attention to detail and ability to prioritize tasks.
