About The Position

Job Title : Physical Therapist

Hours per Shift : 8 : 00 AM - 4 : 30 PM

Experience : One year of experience caring for a similar patient population

License : Licensed as a PT by the state in which practicing

Certifications : Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE)

Must-Have :

  • Ability to work as a collaborative team member
  • Ability to understand and apply guidelines, policies, and procedures
  • Ability to navigate and perform basic use of Microsoft Office products
  • Reliable mode of transportation and ability to drive safely during all day and night hours in all types of inclement weather

Description :

The Physical Therapist (PT) uses clinical expertise, physical assessment, teaching, and skilled therapy interventions to deliver high-quality patient care in the patient’s place of residence.

The PT develops or contributes to the development of the home health plan of care under the direction of the physician.

The PT delivers therapy intervention and treatment directed by the physician as established in the home health plan of care.

The PT ensures quality and safe delivery of services to patients according to the care plan.

The PT performs initial and comprehensive assessments on all patients in accordance with agency policies and procedures.

The PT evaluates and assesses the patient’s level of functioning by applying appropriate tests.

The PT treats clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical therapy plan of care.

The PT initiates diagnostic, preventive, and rehabilitative procedures as appropriate to the patient’s care delivery.

The PT promotes patient independence by teaching patients and family members about treatment and disease management, medications, proper use of equipment, and emergency preparedness.

The PT provides patient, caregiver, and family counseling and education as indicated in the plan of care.

The PT develops and continually updates a plan of care that specifies the care and services required to meet patient-specific needs determined during the comprehensive assessment.

The PT assesses the need for plan of care updates at every visit and communicates care plan revisions to the patient, representative, caregiver, and all physicians involved.

The PT maintains accurate and timely documentation of clinical records and time / travel entries.

The PT is responsible for obtaining physician orders prior to the initiation of care and notifying the physician of any changes in the patient’s condition.

The PT identifies patient risks for hospitalization and proactively intervenes to prevent adverse events.

The PT supervises home health aide and physical therapy assistant staff and documents supervision in the patient medical record.

The PT participates in clinical review of records to monitor quality and accuracy, appropriate utilization, and coordination of care.
