Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

An opportunity is available for a skilled Home Health Registered Nurse (RN) to deliver patient-centered nursing care within a home environment. This role requires a self-motivated and organized professional able to work independently while upholding high standards of clinical excellence and effective patient communication.

Responsibilities include conducting thorough assessments using the OASIS tool, developing and managing personalized care plans, and coordinating with interdisciplinary teams to support positive patient outcomes. The nurse will provide essential skilled care such as wound management, medication administration, IV therapy, and chronic disease monitoring. Additional duties involve educating patients and families, maintaining accurate documentation in the Home Care Home Base (HCHB) electronic system, and ensuring adherence to regulatory standards.

Key Qualifications or Desired Experience :

• Active RN license in good standing within the state
• Minimum one year of recent home health experience; field experience preferred
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Strong clinical assessment, critical thinking, and communication skills
• Reliable transportation with valid drivers license and insurance
• Ability to manage a flexible, independent field-based schedule
• Bachelor of Science in Nursing (BSN) preferred
• Familiarity with Medicare and home health compliance standards is a plus

Schedule / Hours :

Monday through Friday, 8 : 00 AM to 5 : 00 PM

Contract Details :

Full-time, 13-week contract

Caseload : 2532 points per week, typically 35 patient visits daily

Field-based work requiring travel within the assigned territory (mileage reimbursed)
