Description

Southeast Healthcare's Delaware office is on the lookout for an outstanding Therapist to join our organization. Key Responsibilities :

• Assessment and Diagnosis : Conduct thorough assessments and evaluations of clients to understand their mental health needs and develop appropriate treatment plans.
• Therapy and Counseling : Provide individual, group, and / or family therapy sessions to address various mental health concerns.
• Treatment Planning : Develop and implement individualized treatment plans that align with the client's needs and goals, utilizing evidence-based therapeutic approaches.
• Progress Monitoring : Regularly monitor client progress and adjust treatment plans as needed, ensuring clients are making positive strides toward recovery.
• Collaboration : Collaborate with other mental health professionals, such as psychiatrists, social workers, and case managers, to provide comprehensive care.
• Documentation : Maintain accurate and thorough records of client assessments, therapy sessions, and treatment progress.
• Crisis Management : Respond effectively to mental health crises and ensure client safety.
• Case Management : Coordinate and manage client care, including referrals to other services and resources.
• Screening and Evaluation : Conduct intake screening, evaluation, and triage.
