Provides medical services within the scope of licensing, training, and credentialing.

Minimum Education

Graduate of Accredited medical school with degree in Doctor of Medicine (MD) or Doctor of Osteopathy (DO).

Completion of Accredited Residency.

Board Certified or Board Eligible in Specialty (if newly graduated from residency, Board Certification must be obtained within 2 years of graduation) must be kept current.

Unrestricted license to practice within the State of Kentucky.

Current DEA certificate without restriction.

PALS or NRP, ACLS may be required.

Screening Requirements :

• Drug Screen
• Tuberculosis Test
• Background Check
• Physical Exam
• Respirator Fit
