Job Description & Requirements Specialty : Care Manager Discipline : RN Start Date : 10 / 20 / 2025 Duration : 13 weeks 40 hours per week Shift : 8 hours, days Employment Type : Local Contract Integrated Case Manager – Inpatient (RN or LMSW) Location : Detroit, MI Work Type : Onsite / Contract (10 / 14 / 2025 – 01 / 10 / 2026) Experience Acute inpatient case management : Must have at least 5 years , with recent experience within the last 2 years . Experience in discharge planning and care transitions . Exposure to complex patient populations (multiple comorbidities, high-risk cases). Optional but valuable : home health, rehab, community health, or managed care experience . Epic EMR or other hospital EMR experience . Education Nursing degree (BSN preferred if converting to full-time later) OR Master’s in Social Work (MSW) for LMSW candidates.
