Heritage Health Network is hiring a Medical Assistant (MA) to support Transitions of Care (TOC) activities. The MA will help ensure members receive timely follow-up after hospital or facility discharges by assisting with scheduling, outreach, and documentation. This position plays a vital role in reducing gaps in care and improving the member experience.
Key Responsibilities :
- Assist with post-discharge follow-up calls and care coordination
- Schedule follow-up appointments with providers and specialists
- Support medication reconciliation by gathering information for clinical review
- Document outreach and scheduling in ECW accurately
- Communicate with members and families to support smooth transitions
- Provide administrative and clinical support to the TOC team as needed
Requirements
Current Medical Assistant certification (California) requiredPrevious experience in care coordination or clinical support preferredStrong communication and organizational skillsBilingual (English / Spanish) preferred but not requiredBenefits
