This full-time position is based within the bustling and supportive settings of Cuyahoga County, specifically focusing on the Cleveland Eastside. Duties and Responsibilities
- Develop and implement comprehensive care plans tailored to the specific needs of each patient.
- Collaborate with physicians, social workers, chaplains, and other professional staff to ensure the coordination of services.
- Provide direct nursing care by evaluating patient health problems and needs.
- Administer nursing care to ill, injured, convalescent, or disabled patients.
- Advice patients on health maintenance and disease prevention or provide case management.
- Supervise and monitor nursing activities of assigned hospice aides.
- Facilitate family meetings to discuss end-of-life care preferences and plan accordingly.
- Document patient's condition and nursing needs accurately and in a timely manner.
- Adjust care plans as necessary to ensure optimal patient outcomes.
- Communicate with family members and provide updates and education on patient care and management.
- Manage pain and other complex symptoms effectively, enhancing comfort for patients.
- Lead initiatives for improving the quality of care and patient satisfaction.
Requirements
Current Registered Nurse (RN) license in the state of operation.Minimum of three years of experience in clinical nursing, with at least two years in a hospice or palliative care setting.Demonstrated leadership potential, with abilities in team management and decision-making.Exceptional bedside manner with a proven track record in empathetic patient care.Advanced skills in pain and symptom management, with a deep understanding of palliative care principles.Effective communication skills, including both verbal and written proficiency.Strong organizational abilities and a high level of attention to detail.Commitment to the ethical guidelines of the nursing profession, particularly in a hospice environment.Ability to work under pressure in a fast-paced setting.Mandatory participation in continuous education and professional development.Flexibility in working hours, as patient needs can often extend beyond standard working hours.Membership or eligibility for the ONA Union.
