Job Summary
We are seeking a Licensed Practical Nurse (LPN) to join our team in the Ambulatory Care setting. Under general supervision, the LPN will provide preventative and health care services to an assigned group of patients or clients in an Client facility. The ideal candidate will have strong clinical skills, a compassionate approach to patient care, and the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.
Key Responsibilities
- Administer medications safely under the direction of a physician or other medical personnel.
- Observe, record, and report the patient's condition and reaction to drugs and treatments to physicians.
- Provide care according to an established nursing plan and adjust as needed based on patient progress.
- Collect comprehensive data pertinent to the patient's health or situation, ensuring complete and accurate documentation.
- Ensure accurate and timely chart entries and appropriately forward patient charts to other team members.
- Communicate systematic reports to other care providers, ensuring smooth transitions in patient care.
- Coordinate care delivery and utilize available resources to promote the continuum of care for patients.
- Employ strategies to promote health in a safe and supportive environment.
- Document progress towards the attainment of patient outcomes and goals.
- Use quality and evidence-based practices to deliver and improve care.
- Work closely with more experienced clinicians for guidance and supervision.
- Triage telephone calls and make appropriate referrals to other clinical team members.
Required Qualifications
Experience :
At least one year of experience in a healthcare setting.Knowledge of professional nursing theories, practices, techniques, and procedures.Ability to organize, coordinate, plan, and evaluate nursing needs for assigned patients.Surgical Experience : Must be knowledgeable about operating room techniques if applying for a surgery-related position.Skills :
Strong verbal and written communication skills.Excellent presentation skills for both internal and external communication.Ability to work effectively in a team-oriented environment.Certifications :
Current LPN License issued by the Oklahoma State Board of Nursing or a current multistate compact LPN License (eNLC).Basic Life Support (BLS) certification from the American Heart Association.Preferred Qualifications

Two or more years of experience in an ambulatory care or related healthcare setting.Certifications :
Additional certifications related to nursing practice or ambulatory care may be beneficial but are not required.Certifications
