River Edge offers behavioral health outpatient services to adults in Macon-Bibb County experiencing mental health and substance use challenges and is looking to fill the position of Admissions Care Coordinator with / at our Bibb Outpatient Services program.

In this role you will :

Provide care coordinator of individuals receiving services through River Edge Behavioral Health to ensure that individuals' behavioral health, physical health, and other identified needs are met and that services are coordinate with primary care and other systems with which the individuals is involved. Primarily interfaces with the Admissions Clinicians to ensure appropriate referrals and coordination is maintained with internal and external service providers.
