  • A caregiver in this position works 8 : 00AM - 4 : 30PM. \n\nA caregiver who excels in this role will : \n\n+ Ensure compliance with standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation.\n\n+ Promote interprofessional synchronization throughout the acute episode.\n\n+ Complete effective handoffs to the next level of care or community to ensure continuity of care.\n\n+ Proactively assess, identify and work to mitigate the risk posed by barriers to patients' transitions of care.\n\n+ Work with the patient and / or care partner(s) to develop a comprehensive, individualized transitional care plan.\n\n+ Participate and lead in patient rounding activities, huddles and committee meetings.\n\nMinimum qualifications for the ideal future caregiver include : \n\n+ Bachelor of science in nursing (BSN)\n\n+ Current state licensure as a Registered Nurse (RN)\n\n+ Basic Life Support (BLS) certification through the American Heart Association (AHA)\n\n+ 3-5 years of recent RN clinical experience\n\n+ Working knowledge of MCG guidelines\n\nPreferred qualifications for the ideal future caregiver include : \n\n+ Certification by an approved entity within one year of eligibility (CTM, ACM, CCM or AHA)\n\n+ 2 years of recent experience in utilization review, quality or care management\n\n+ Knowledge of acute care and accreditation requirements\n\n+ Experience in discharge planning and quality improvement\n\nOur caregivers continue to create the best outcomes for our patients across each of our facilities.
