This will be a Part Time role, every other Friday and Saturday. Compensation includes a $5,000 Sign On Bonus. We offer competitive salaries, full benefits package, Paid Time Off, and opportunities for professional growth.

Responsibilities

• Interface with the clinical team in decision-making process involving patients, including attending all clinical meetings.
• Assess the counseling needs of the patient.
• Assess the patient for substance abuse diagnosis using DSM criteria and determine the appropriate level of care based on ASAM criteria.
• Obtain previous records relevant to the current treatment episode.
• Prepare and provide the substance abuse counseling portion of the patient treatment plan; deliver counseling services specified in the plan.
• Develop the patient discharge / continuum of care plan to ensure movement to appropriate levels of care.
• Participate as a member of a multidisciplinary team for assigned patients.
• Document all counseling, education services, assessments, referrals, and follow-up in the patient's clinical record.
• Maintain patient caseload and perform case management, utilization review updates, treatment planning duties, and clinical charting as required.
• Coordinate support services for the patient and family and record the services provided.
• Reassess patients and revise the counseling portion of the treatment plan as necessary.
• Facilitate family therapy as assigned.
• Coordinate with referral sources (e.g., legal, school, employer).
• Facilitate crisis-oriented interventions using a multidisciplinary approach.
• Attend team meetings and complete required training in a timely manner.
• Other duties as assigned.
