Position Overview :
The Guia (Patient Concierge) is a trusted partner who provides comprehensive care and social support coordination to a dedicated panel of patients. You will work collaboratively with patients, their families, and our multidisciplinary care team to navigate the complexities of the healthcare system and address critical social needs. This role is a unique blend of case management, patient education, and relationship-building, focused on empowering patients to achieve their best possible health outcomes.
What You'll Do (Responsibilities) :
Patient Advocacy & Care Coordination :
Serve as the primary point of contact for an assigned caseload of patients, guiding them through our unique care model from onboarding to ongoing support.
Seamlessly coordinate care across the continuum, including scheduling provider appointments, managing referrals, and ensuring smooth information transfer between internal and external care teams.
Collaborate with providers, Transitions of Care managers, and Medicaid case managers to create a truly integrated and beneficial care experience.
Relationship Building & Support :
Establish healing, trust-based relationships with patients and their families, intervening with empathy to address the emotional, social, and financial consequences of illness.
Employ confidence-promoting techniques to develop patient self-efficacy and empower them to better manage their health.
Provide essential education to patients on the management of acute and chronic diseases.
Communicate with patients through their preferred methods, including in-person meetings, phone calls, video conferences, and text messaging.
Community & Home-Based Care :
Conduct safe and professional in-person visits to patients' homes to better understand their environment and needs.
Identify, access, and mobilize community and family resources to meet complex social care needs (e.g., transportation, food security, housing).
Maintain expert knowledge of Medicare, Medicaid, and other programs to help patients navigate benefits and make informed choices.
Team Collaboration & Operations :
Meticulously document all patient interactions and interventions in the electronic medical record in a timely and accurate manner.
Actively participate in the daily operations of a primary care practice, assisting with answering phones, managing messages, and maintaining the clinical space.
Track patient enrollment and progression through care programs, providing insights to the Guia Manager and care team.
Serve as a resource and consultant to other team members on patients with significant community resource needs.
What You'll Bring (Qualifications)
Bilingual fluency in English and Spanish is required.
Previous experience in a patient-facing role such as a Community Health Worker, Medical Assistant, Case Manager, or Social Work Assistant is highly preferred.
Deep understanding of and empathy for the health and social challenges faced by diverse populations.
Exceptional interpersonal, communication, and relationship-building skills.
Strong organizational and problem-solving abilities with a talent for managing multiple priorities.
Comfortable with technology, including electronic medical records and patient communication platforms.
Must have a valid driver's license, reliable transportation, and proof of auto insurance for home visits.
High school diploma or GED required; Associate's or Bachelor's degree in a related field is a plus.
