A company is looking for a Manager, Community Health ED Navigator.
Key Responsibilities
Manage a team focused on transitions of care for emergency room discharges
Provide clinical oversight and ensure adherence to policies and programming requirements
Evaluate team performance and participate in quality improvement efforts
Required Qualifications
At least 3 years of experience as a Registered Nurse in a high-volume acute care setting
Bachelor's of Nursing from an accredited university
One year of experience with transitions of care or preventing inpatient / ED readmissions
Compact licensed RN in TX or FL
Maintenance of professional credentialing and CME standards
