Job Title : Licensed Social Worker
Duration : 6+ Months
Location : Remote
Location : Standard Hours.
This position will be based from a home office and will travel 15-20% of the time, to an assigned area in Wayne or Macomb County, to conduct in home visits with Medicare / Medicaid members.
Must reside in Michigan with the ability to drive to Wayne or Macomb Counties .
The Social Worker Field Care Manager employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.
Position Responsibilities :
- The Social Worker Field Care Manager will be responsible for managing a case load and completing assessments with members in their home or community-based setting, as well as telephonically.
- Provides clinical support and guidance, collaborates with ICT participants to support members with medical complexity.
- Help develop and coordinate care plans ensuring that patients receive appropriate services to manage their health needs effectively. Addressing barriers to health care and advocating for optimal member outcomes.
- Will review, assess, and complete medical attestations and clinical oversights.
- Ensures members are receiving services in the least restrictive setting to achieve and / or maintain optimal well-being by assessing their care needs.
- Develops and modify Individual Care Plan and involve applicable members of the care team in care planning (Informal caregiver, coach, PCP, etc.).
- Focuses on supporting members and / or caregivers utilizing an interdisciplinary approach in accessing social, housing, educational and other services, regardless of funding sources to meet their needs.
- Primary point of contact for the Interdisciplinary Care Team (ICT) and shall be responsible for coordinating with the member, ICT participants, and outside resources to ensure the member’s needs are met.

Required Qualifications :
Licensed Bachelor's prepared Social Worker or continuous active limited social worker license with intent to obtain full licensure.Minimum of 2 years of post-degree clinical experience in behavioral health settingMichigan State licensure in field of studyMust reside in the state of MichiganAbility to travel to region-based facilities and homes for face-to-face assessmentsExperience working with the adult population, disease management.Knowledge of community health and social service agencies and additional community resourcesExceptional communication and interpersonal skills with the ability to quickly build rapportAbility to work with minimal supervision within the role and scopeAbility to use a variety of electronic information applications / software programs including electronic medicalrecords
Excellent keyboard and web navigation skillsIntermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and ExcelAbility to work full-time (40 hours minimum) Mon-FriRequires an individual to have a valid statedriver's license and proof of personal vehicle liability insurance with at least 100 / 300 / 100 limits
