Direct Care Aide :

• CNA, PCA, DCA, NA Position Summary :   Provide personal care services, assistance, and support to resident while upholding service standards, philosophy, and mission.

Position Responsibilities – Service Following the guidelines as outlined in the resident care plan (ISP), assist residents with activities of daily living including bathing, dressing, eating, hygiene, and mobility Monitor the health, safety and well being of all residents, maintaining a daily communication log; document information pertaining to changes in residents’ functional status and personal needs Cleaning and trimming finger and toe nails Wash, Dry, Fold / Match resident laundry Strip and make beds Assist in serving meals and bussing tables Participate in and attend all required in-services training sessions Answer business phones Tour inquiring individuals Clean up urine , bowel movements, or blood when housekeeping is unavailable Answering call bells and respond to alarms All other duties assigned Team Effort :

Name tag Watch with second hand and black pen Hair should be out of the way of care No piecing that dangle.  No lip or eyebrow rings Physical Demands and Work Environment :
