• Design and implement patient-specific care plans focused on achieving optimal recovery and enhancing overall health.\n
• Provide high-quality nursing care in alignment with the patient's care plan, including comprehensive health and psychosocial assessments to support recovery.\n
• Monitor patients' conditions and coordinate health promotion, prevention, and direct care.\n
• Maintain detailed and accurate clinical records, including all required OASIS documentation.\n
• Communicate effectively with clinical managers and colleagues to ensure high-quality care.\n
• Provide training and support to patients and their families / caregivers.\n
• Function as a key point of contact between patients and community health resources.\n
• Work alongside physicians and discharge planners to provide health education to patients and caregivers.\n
• Confirm daily visit schedules to ensure seamless coordination with other staff.\n
• Demonstrate awareness of the agency's Quality Assurance and Performance Improvement (QAPI) program and participate as required.\n
• Understand and adhere to the agency's process for managing and addressing customer complaints.\n
• Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency's service agreements and admission policies.\n
• Understand the agency's policies regarding appropriate patient discharge and reasons for discharge.\n\nQualifications\n
• Must have a current RN license (state-specific).\n
• Valid BLS certification.\n
• Minimum one year of experience in home health or a healthcare facility.\n
• Familiarity with Medicare and Medicaid guidelines.\n
• Possess a solid understanding of professional nursing principles and home health documentation.\n
• Excellent communication, organizational, and time management skills.\n
• Must have a valid driver's license and access to a reliable and insured vehicle.\n
• Ability to visit homes that may not be wheelchair accessible.\n
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.\n
