HomeWell Care Services Care Manager

At HomeWell Care Services, our Care Managers don't just supervise care they inspire confidence, support caregivers, and ensure our clients receive the compassion and dignity they deserve.

This is the role for you if you're ready to :

  • Be on the move visiting clients and caregivers across multiple locations (reliable transportation required).
  • ? Step in when it counts filling caregiver shifts when needed to ensure clients never go without care.
  • Lead with flexibility be on-call every other weekend to support caregivers and families.
  • Be a trusted partner building strong relationships with clients, families, and caregivers while keeping care plans on track.

What We're Looking For :

  • At least 2 years of experience in home care, home health, or related field.
  • Reliable transportation, valid driver's license, and current auto insurance.
  • Strong communication and leadership skills.
  • Compassion, professionalism, and problem-solving ability.
  • Comfortable stepping in as a caregiver when needed.

    • What You'll Do :

  • Develop and oversee personalized care plans.
  • Train, mentor, and supervise caregivers.
  • Visit clients regularly to ensure needs are met.
  • Respond quickly to client and caregiver concerns.
  • Keep accurate documentation and ensure compliance.

    • Salary Range : $40,000 $60,000 annually, based on experience and qualifications.

    Schedule : Full-time, with on-call duties every other weekend.
