Social Worker (SW)

Location : Aristos Nursing & Rehabilitation Center – Cleveland, OH

Type : Full-Time

Aristos Nursing & Rehabilitation Center is seeking a compassionate and motivated Social Worker to join our care team. The Social Worker plays a key role in supporting residents and families through the admission, care, and discharge process while ensuring emotional and social needs are met.

Responsibilities

Assist residents and families with care planning, adjustment, and discharge needs.

Coordinate with nursing and interdisciplinary teams to support residents’ psychosocial well-being.

Maintain required documentation and ensure compliance with state and federal regulations.

Advocate for residents’ rights and provide guidance and support during transitions.

Qualifications

Bachelor’s degree in Social Work (BSW) required; LSW preferred.

Experience in a skilled nursing or healthcare setting strongly preferred.

Excellent communication, organization, and interpersonal skills.
