Job Title : Field Case Manager (Registered Nurse)
Location : Kalamazoo, MI or Grand Rapids, MI
Schedule : Monday - Friday, 9 : 00 AM - 5 : 00 PM
About the Role
We're seeking an experienced Field Case Manager (RN) to join our team and provide hands-on case management and medical coordination services for clients recovering from auto or work-related injuries. This role combines clinical expertise, compassionate client support, and strong organizational skills to ensure each client receives comprehensive, timely, and effective care.
Travel is required within the local service area for client visits and care coordination meetings.
Key Responsibilities
Case Intake & Case Management
Attend client appointments and meetings within the assigned service area.
Receive and process referrals from insurance companies, physicians, or direct clients.
Create and maintain accurate, organized digital and physical case files.
Ensure all documentation meets compliance and reporting standards.
Medical Coordination & Client Support
Assess the medical, emotional, and rehabilitation needs of auto injury clients.
Coordinate treatment plans, including physical therapy, diagnostic tests, and specialist consultations.
Act as a liaison among clients, providers, and insurers to ensure clear communication and continuity of care.
Monitor treatment progress, medication, and therapy adherence.
Arrange medical supplies, home modifications, and rehabilitation equipment as needed.
Educate clients and families about recovery processes and treatment options.
Reporting & Documentation
Prepare detailed case progress reports for insurers or clients.
Maintain documentation for billing, compliance, and reporting.
Use case management software to track client status and follow-up tasks.
Collaboration & Compliance
Work closely with insurance adjusters and healthcare professionals to coordinate care and claim documentation.
Ensure adherence to workers' compensation regulations and internal policies.
Support billing and claims review processes.
Qualifications
Active Registered Nurse (RN) License - required.
3-5 years of case management experience in medical, rehabilitation, or insurance settings.
Strong clinical background with the ability to assess and coordinate complex care needs.
Excellent communication, organizational, and documentation skills.
Must be comfortable with fieldwork and regular travel within the service area.
Proficiency with case management or EMR software preferred.
Schedule & Compensation
Full-time, Monday-Friday, 9 : 00 AM-5 : 00 PM
Competitive compensation based on experience.
