job_description.job_card.job_descriptionPerform initial and ongoing clinical assessments to identify patient needs, and determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home health Develop and revise a Plan of Care in consultation with the physician and other care team members Provide treatments, evaluate the need for and use of adaptive equipment and environmental modifications, fabricate / maintain adaptive equipment and hand splints as applicable Evaluate outcomes of care with appropriate discharge planning Apply concepts of infection control and standard precautions in coordinating / performing patient care activities to prevent contamination and transmission of disease Accept clinical assignments that are consistent with education and competence to care for patients Supervise COTA and HHA as appropriate. Complete appropriate documentation in a timely manner Meet mandatory continuing education requirements of the agency / licensing board Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate Demonstrate commitment, professional growth, and competency Promote agency philosophy and administrative policies to ensure the quality of care Education : Bachelor of Science in Occupational Therapy Licensure : Current Texas State License as an Occupational Therapist, current Texas Drivers License Experience : Two years experience as an Occupational Therapist in an acute or rehabilitation setting Skills : Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of CPR and Hepatitis profile. Knowledge of durable medical equipment Transportation : Reliable transportation and valid and current auto liability insurance

- Provides intervention to deficits and elicits responses
- Collaborates with the interdisciplinary team.
