Responsibilities :
The role of the Longer-Term Housing Case Manager is to provide and coordinate continued supportive aftercare housing, counseling services, and advocacy to a caseload of clients / former shelter residents in order to enhance their ability to maintain safe independent living for successful integration into the community in accordance with the mission and vision of the Agency.
