What You Will Do as a Home Health Clinical Manager (RN)
As a Stonerise Home Health Clinical Manager (RN), you will manage a multidisciplinary team to provide outstanding patient care to achieve the highest quality outcomes. You will oversee patient care plans and ensure the appropriate care coordination between all disciplines, physicians, and caregivers. Some of your primary responsibilities include :
- Auditing charts for clinical oversight, compliance, and adherence to the Medicare guidelines
- Managing clinical activities, including patient assessments, care plan development, service level determination, and on-site field visits
- Directly supervising clinical team and participating in the recruitment, selection, and orientation of team members
- Ensuring service quality and participating in care coordination to ensure proper communication between caregivers, patients, referral sources, etc.
- Discussing operational issues, updating staff on new / changed regulations, and reviewing records to ensure regulatory compliance
- Overseeing the assessment and reassessment of patients, including updating the care plans and interpreting patient needs
- Coordinating communications between team members, attending physicians, and caregivers to ensure appropriateness of care and outcome planning
- Participating in performance improvement activities, and maintaining ongoing clinical knowledge through internal / external training programs
- Identifying clinical problem areas and documenting deficiency trends in a proactive approach
- Organizes Clinical Operations for the care center
- Participates in the on-call process
- Leads Interdisciplinary team meetings
- Assist with scheduling of patients
- Provides Direct patient care as needed.

Education, Licensure & Certification Requirements
Previous Home Health experience of 1-2 years requiredGraduate of an accredited school of nursing.Active WV RN licensureBachelor's degree preferredOne (1) to two (2) years of recent acute care experience in an institutional settingMust be a licensed driver with an automobile that is insured per state / or organization requirements and is in good working order
