The ideal candidate will be proficient in OASIS assessments and familiar with Homecare Homebase software. • Perform patient assessments and develop care plans\n
• Administer prescribed treatments, medications, and wound care\n
• Complete OASIS documentation and manage patient records in Homecare Homebase\n
• Educate patients and their families about disease management and health promotion\n
• Ensure coordination of care through communication with healthcare team members\n\nQualifications : \n
• Active RN license in Oregon\n
• Minimum of 2 years of bedside Home Health experience\n
• Proficient in OASIS documentation and Homecare Homebase\n
• Excellent interpersonal and communication skills\n
