Responsibilities
Provide direct patient care and support under the supervision of registered nurses.
Administer medications and monitor patient responses to treatments.
Assist in developing and implementing patient care plans.
Participate in ongoing professional development and training.
Engage in performance measurement and goal setting.
Maintain accurate patient records and documentation.
Collaborate with healthcare team members to ensure quality care.
Monitor vital signs and report any changes to the nursing staff.
Utilize pipelines to assess potential success in patient care.
Educate patients and families on health management and wellness.
Ensure compliance with healthcare regulations and standards.
Assist with daily living activities for patients as needed.
Experience / Qualifications
Valid LPN / LVN license in the state of practice.
Willingness to engage in ongoing professional development and training.
Experience in a clinical setting with direct patient care.
Ability to monitor and document patient vital signs accurately.
Experience using patient care assessment tools and pipelines.
Strong communication skills for patient and team interactions.
Ability to collaborate effectively with healthcare team members.
Demonstrated ability to educate patients and families on health topics.
Familiarity with healthcare regulations and compliance standards.
Strong organizational skills for maintaining accurate patient records.
