Care Career is seeking a LPN / LVN Long Term Care for a travel job in Penn Yan, New York.

Job Description & Requirements

Requirements :

Active NY LPN License

BLS Certification (AHA or RQI)

Minimum 1 year of LPN experience (preferably in LTC setting)

Position Summary :

The LPN provides direct and indirect skilled nursing care under the supervision of a Registered Nurse. This role involves delivering age-appropriate and culturally sensitive care to adult and geriatric patients while maintaining collaborative relationships with the healthcare team. The LPN is expected to maintain clinical competency through continuing education and actively support a safe and positive patient care environment.

Key Responsibilities :

Collect and report data on patients’ physical and psychosocial condition to the RN and interdisciplinary team.

Contribute to and revise individualized care plans to meet patient outcomes.

Promote a safe, healthy environment for patients, families, and staff.

Participate in mentoring and orientation of staff.

Demonstrate leadership within the scope of nursing practice under RN supervision.

Follow infection control standards; occupational exposure to bloodborne pathogens is reasonably anticipated.

Skills & Competencies :

NY LPN Licensure & BLS – Required

Minimum 1 year of experience as LPN in LTC Facility – Required

Strong communication, assessment, and documentation skills

Ability to work collaboratively in a team environment
