Case Management Job Details

Location : Stroudsburg, PA (80% Monroe, 20% float to Miners) – ideal for candidates seeking Pennsylvania healthcare jobs

Employment Type : Local, On-Site

Contract Length : 13 weeks

Hours : 40 hours per week (5x8s), between 8 : 00 AM – 5 : 00 PM

Weekend Availability : Every other weekend required

Start Date : November 10, 2025

Hourly Pay Rate : $83.70

Estimated Weekly Salary Range : $2,030 - $2,190

Case Manager Qualifications

Minimum 2 years of experience as a Registered Nurse (RN) in care management or as a Social Worker (MSW) in an acute hospital setting

MSW or RN with 2 years of care management experience required

Acute care experience (minimum 2 years) is mandatory

Ability to float between Monroe and Miners locations as needed

Flexibility to work late hours depending on case load

Every other weekend availability

Case Management Responsibilities

Deliver comprehensive case management and clinical support services to patients in an acute care hospital setting

Coordinate individualized care plans and collaborate with interdisciplinary healthcare teams

Support patients and families through transitions of care, discharge planning, and resource referrals

Ensure compliance with hospital policies, healthcare regulations, and quality standards

Float between Monroe and Miners campuses as needed to meet patient care needs
