    • What You'll Do

  • Deliver comprehensive outpatient primary care to a defined patient panel.
  • Manage chronic conditions, preventive care, and early interventions.
  • Work with care teams to improve HEDIS, continuity-of-care, and quality metrics.
  • Reduce avoidable ER visits and hospitalizations through proactive care planning.
  • Create personalized care plans for high-risk or complex patients
  • Use clinical dashboards and EHR systems to track outcomes and guide care.
  • Patient population : Approximately 80% Medicaid, 10% Medicare and 10% fee-for-service, with a strong focus delivering high quality care to underserved and vulnerable communities

    • What You'll Bring

  • MD or DO, Board Certified or Eligible in Family Medicine
  • Active Florida medical license (or eligibility)
  • 2+ years of experience in outpatient or value-based care (preferred)
  • Strong commitment to serving underserved populations
  • Comfortable using data and working in team-based care models
