Occupational Therapist - Patient Care Specialist

We are seeking a compassionate and skilled Occupational Therapist to provide patient-centered care in the home environment. As an OT, you will be responsible for evaluating patients' occupational needs and developing treatment plans to help them achieve their goals.

Job Description Evaluate patients' occupational needs and develop personalized treatment plans.

Provide one-on-one therapy services, including assessments, interventions, and education.

Collaborate with interdisciplinary care teams to ensure seamless patient care.

Maintain accurate records of patient progress and outcomes.

Required Skills and Qualifications Master's degree in Occupational Therapy or related field.

Active state licensure in Occupational Therapy.

Minimum 2 years of experience in acute or post-acute care settings.

Excellent communication and interpersonal skills.
