Minimum Years of Experience : 2 years of acute care experience

License : Current Registered Nurse license

Certifications : Relevant certifications required

Must-Have :

Acute care hospital experience in both Utilization Management and Care Coordination (discharge planning)

Ability to rotate every other weekend

Experience with case loads up to 25 patients

Experience in arranging Durable Medical Equipment (DME), Skilled Nursing Facility (SNF), and ambulance transport

Description :

The role involves managing patient care from admission to discharge.

Experience with admission criteria, appeals, and denials is required.

Familiarity with behavioral health, concurrent and retrospective reviews is important.

Knowledge of CMS guidelines and determining medical necessity per evidence-based guidelines is essential.

Competence in coding and billing (CPT and ICD 10) is necessary.

Understanding regulations such as HIPAA, OSHA, and NCQA is expected.

Participation in interdisciplinary team meetings and patient care discussions is required.

Candidates should have a background in discharge planning and benefits eligibility.

Support services will include physical therapy, social services, pharmacy, and rapid response teams.

Applicants should demonstrate the ability to work collaboratively with various healthcare professionals.

The position requires flexibility to float between facilities as needed.

Providers must commit to working three of the five major holidays.

Minimum two supervisory references are necessary covering relevant experience within the last three years.

Candidates must complete a skills checklist prior to consideration for the position.
