Nature & Scope : Positional Overview
The Care Advocate (Community Health Worker) will play a vital, patient-facing role in supporting health and independence for members in the community. This field-based position involves conducting in-home visits, assisting with care transitions, and addressing social determinants of health (SDOH) to connect patients with the resources they need. No direct hands-on clinical care is required, but the role requires daily face-to-face interactions with patients.

- Conduct in-home visits to members to assess needs, provide support, and promote health and wellness.
- Assist with care transitions from hospitals or other facilities to ensure smooth continuity of care.
- Identify and address social determinants of health (e.g., housing, food access, transportation, social support).
- Build trusting relationships with patients and act as a liaison between patients, providers, and community resources.
- Support patients in navigating healthcare systems and connecting with appropriate services.
- Document visits and patient progress in accordance with program requirements.
- Travel within a 25–30-mile radius daily; mileage reimbursed.

Qualifications That Will Help You Thrive
High School Diploma or GED.California-recognized Community Health Worker (CHW) certification (programs typically 6–12 months in duration).Strong interpersonal and communication skills.Ability to work independently in the field and manage time effectively.Reliable transportation, valid driver’s license, and ability to travel within designated territory.Previous experience in community health, social work, or patient advocacy role.Familiarity with Orange County or Inland Empire healthcare and community resources.
