Nature & Scope : Positional Overview

The Care Advocate (Community Health Worker) will play a vital, patient-facing role in supporting health and independence for members in the community. This field-based position involves conducting in-home visits, assisting with care transitions, and addressing social determinants of health (SDOH) to connect patients with the resources they need. No direct hands-on clinical care is required, but the role requires daily face-to-face interactions with patients.

• Conduct in-home visits to members to assess needs, provide support, and promote health and wellness.
• Assist with care transitions from hospitals or other facilities to ensure smooth continuity of care.
• Identify and address social determinants of health (e.g., housing, food access, transportation, social support).
• Build trusting relationships with patients and act as a liaison between patients, providers, and community resources.
• Support patients in navigating healthcare systems and connecting with appropriate services.
• Document visits and patient progress in accordance with program requirements.
• Travel within a 25–30-mile radius daily; mileage reimbursed.
