This is a supervisory position with a combination of case management and direct care, with a high emphasis on managing staff. Candidates should enjoy managing people and serving the disability community. Candidates must pass a drug screen.
Position Summary The Program Coordinator is responsible for supervising Skills & Activities Providers and Persons Receiving Services in the day program. Duties include assisting in setting and upholding program standards, organizational development, performance management, policy recommendation, and overall case management. The Program Coordinator is a representative of Our House Inc. and is expected to professionally represent the organization’s mission and purpose . Responsibilities Meet all the responsibilities of the Skills and Activity Providers; work with Persons Receiving Services (PRS) in providing direct care, as needed.
Main point of contact for PRS, their caregivers, and contracting agencies for PRS specific issues, including daily agency reports and other paperwork.
Oversee PRS programming to ensure it is meeting specific program criteria.
Be willing to go between day program sites to provide support as needed to S&A Providers.
Establish and maintain guidelines by which S&A Providers, PRS, and their caregivers can follow in order to give and receive the best services and growth opportunities possible.
Meet with S&A Providers to review paperwork, programming quality, concerns, and opportunities and provide evaluations as necessary.
Assist in training and retraining of S&A Providers to ensure knowledge of expected job duties, as necessary.
Address and work to resolve any questions, comments, and complaints submitted by employees, students, clients, or guests of Our House Inc.
Attend, contribute, and participate in supervisor meetings to advance the agency, PRS’, and employees’ potential and opportunity.
Participate in disseminating information up and down the chain of command; demonstrate clear communication both written and orally.
Review SP goals, incident reports, create and update goals, ISSP’s, BISSP’s, and other paperwork Create, organize, and maintain PRS profiles and comprehensive files.
Provide agency tours to potential students, families, and contracting agencies, as needed.
Attend IDT’s, Annual staffings, IEP meetings, HRC reviews and other meetings.
Assist in facilitating tasks for Service Adventure Interns and other volunteers.
Oversee medication administration and review regularly to ensure administration meets requirements.
Review PRS activity funds, send home receipts / invoices, and address any found deficiencies.
Maintain the productivity and progress of daily activities through implementation, direction, and / or assistance in goal / activity setting and accomplishment.
Advise S&A Providers on agency, state, and DD rules and regulations to ensure compliance.
Perform general clerical duties to include but not limited to :
Superior critical problem-solving skills and decision-making skills.
