Title : Social Care Network Supervisor

Reports To : VP of Program Development and Implementation

FLSA : Non-Exempt

Status : Full-time

Supervisory Responsibility : SCN Screeners and Navigators

Purpose of the Role :

Responsible for managing the team of Social Care Navigators and screeners who will be providing outreach and health related service delivery (HRSN) to eligible Medicaid members. The appointed individual will supervise the referral and enrollment procedures, and support SCN Navigators in addressing challenges that may impact participants' eligibility for health-related programs.

This role involves supervision, training, quality assurance, and collaboration with other agencies and stakeholders

Essential Functions of the Role :

  • Supervise and support SCN Navigators and Screeners, ensuring compliance with the SCN’s program policies and procedures
  • Coordinate team schedules, manage caseloads, and set productivity targets for screening, navigation and HRSN contacts
  • Oversee service delivery invoices for accuracy, timeliness, and regulatory compliance
  • Serve as the primary point of contact for the SCN program; represent the agency in internal and external meetings
  • Ensure services meet client needs and program goals
  • Identify gaps and strategic priorities across teams; collaborate to develop and implement solutions
  • Provide training, coaching, and development opportunities to enhance staff skills
  • Monitor program performance; prepare and submit timely reports, data, and documentation
  • Communicate programmatic updates and needs to senior leadership
  • Special projects and other duties as assigned

Work Schedule :

Monday – Friday, 9am – 5 pm, 35 hours per week, however flexibility required to address client and Agency needs.

Physical Environment :

o Traditional office environment.

o Must be comfortable attending appointments at various agencies, facilities, and clients’ homes.

Qualifications for this Role :

  • Bachelor’s degree in social work, Public Health, Human Services, or related field (Master’s preferred)
  • Minimum 3 years of supervisory or leadership experience in healthcare or social services settings
  • Experience serving Medicaid populations and addressing Health-Related Social Needs (HRSN)
  • Strong leadership, communication, and organizational skills
  • Proficiency in reporting, documentation, and quality assurance
  • Bilingual or multilingual abilities are a plus.
  • Compensation range : $46,000-$48,000
