Full-time, 8-hour day shifts (M–F, 40 hrs / week) at a large academic Level 1 trauma hospital. Responsible for inpatient discharge planning and coordination across multiple units.

Requirements :

RN with 2+ years acute care case management / discharge planning experience (6+ months in a large academic or Level 1 trauma center)

Recent EPIC experience required

BLS required; CCM preferred

Must be available for on-camera interview within 72 business hours of submission

No time off during Christmas

Details : 50-mile radius rule

Caseload : 15–20 patients / day

Works closely with social work and interdisciplinary teams

Business or professional attire or any color scrubs

Reliable for full 5x8 schedule, no exceptions
