Your clinical expertise will be key in implementing evidence-based nursing practices throughout the care continuum—including assessment, planning, implementation, evaluation, and outcomes. - Conduct thorough nursing assessments tailored to individual patient needs, recognizing changes in patient status.\n
- Create and implement personalized care plans in partnership with patients, families, and the healthcare team.\n
- Continuously update care plans based on patient condition and collaborate with team members regarding changes.\n
- Set realistic and measurable goals for patients, involving them and their families in the planning process.\n
- Respect and incorporate the cultural, spiritual, and personal beliefs of patients and families into care planning.\n
- Plan for patient discharge to ensure a smooth transition to the next level of care, including appropriate referrals.\n
- Provide direct nursing care in alignment with established plans and policies.\n
- Delegate tasks to healthcare team members as appropriate while ensuring task completion.\n
- Monitor patient quality of care, ensuring standards and protocols are consistently followed.\n
- Coordinate care efficiently, utilizing resources effectively and incorporating cost containment principles.\n
- Ensure a safe environment for patients by adhering to established safety protocols and patient care standards.\n
- Conduct performance monitoring and promote quality improvement initiatives while evaluating nursing care effectiveness.\n
- Assess and address patient / family educational needs with clear, understandable language and supportive examples.\n
- Document patient care and education efforts clearly, ensuring continuity of communication within the healthcare team.\n
- Act as a patient advocate while coordinating care among healthcare providers and team members.\n
- Engage in ongoing professional development and maintain licensure, reflecting on personal practice for continuous improvement.\n
- Perform additional duties as assigned, contributing to a team-oriented work environment.\n\nSupervisory / Management Responsibility\n\nThis position does not involve management responsibilities and reports to a supervisor or manager.\n\nMinimum Requirements\n
- Associate degree in Nursing is required; Bachelor's degree in Nursing is preferred.\n
- One (1) year of experience as a Registered Nurse is required.\n\nIn Lieu Of\n
- Successful completion of the Prisma Health Nurse Residency Program or a comparable residency may substitute for experience.\n
- Internal candidates may substitute one year of licensed practical nurse (LPN) experience in the same service line for RN experience.\n
- An accepted nursing diploma with licensure may substitute for an Associate degree in Nursing.\n\nRequired Certifications, Registrations, Licenses\n
