Transition Coordinator

From birth to end of life, Lifelong Access provides a continuum of services and supports for families, children, teens, and adults including residential housing, vocational development, supported employment, high school transition, behavioral health counseling and services, and a wide variety of pediatric therapy supports.

Guided by our Core Values, our team members have a strong drive and big hearts for improving the lives of others; a place where we support each other and the individuals who put their trust in us.  Lifelong Access is seeking a   Transition Coordinator to   support  the transition of young adults from high school into adult services including vocational and residential services.  This position will serve as the Lifelong Access liaison to the local school systems.   Pay :

• $24 per hour  Job Type :              Full-time Primary responsibilities include : Communicate and serve as an agency liaison to all school systems, families, guardians, people we support, peers, community, neighbors, work groups and any other potential business partners.  Pursue and follow up with families interested in employment initiatives including, but not limited to :   initial presentation, follow up with families, assessment and evaluation of potential candidates, maintaining open lines of communication between potential student, family members, school counselors and Lifelong Access program directors.  Provide leadership to all staff regarding quality of life for people supported.  Develop and maintain natural support systems for people.  Participate in the IEP planning process for success of the students supported.  Recommend and develop where feasible educational, vocational, and recreational programs for students.  Support grant application and implementation.  Participate in agency-wide functions to support entire organization, including intra-agency and inter-agency committees and development of policies and procedures.  Qualifications for this position include : Bachelor’s Degree in a human service field required  Transition experience preferred  One year of experience supporting people with disabilities  Ability to be placed on the QIDP Registry  Ability to work with a flexible schedule to assist those supported, with occasional weekends and evenings  Valid Illinois driver’s license with driving record in good standing  Current proof of automobile insurance  Working Conditions          While performing the duties of this job, the employee is occasionally exposed to outside weather conditions.  The noise level in the work environment is usually moderate Physical Requirements : The physical demands described here are representative of those that must be met by an employee to successfully perform essential functions of this job.
