Job Summary :

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse or physician. The LPN ensures that all resident care is provided in a safe, compassionate, and effective manner that complies with facility policies, federal and state regulations, and standards of nursing practice.

Key Responsibilities :

Administer medications and treatments as prescribed by physicians.

Monitor residents for changes in condition and report findings to RN or physician.

Provide direct patient care including wound care, catheter care, and assistance with ADLs (activities of daily living).

Document nursing care, observations, and interventions accurately and timely in the medical record.

Assist with admissions, transfers, and discharges of residents.

Follow infection control and safety procedures.

Communicate effectively with residents, families, and interdisciplinary team members.

Participate in care plan meetings and contribute to the development of individualized care plans.

Supervise CNAs and ensure proper delivery of care in accordance with residents’ care plans.

Respond to emergencies and provide basic life support as needed.

Qualifications :
