Must-Have :

Reliable, independent means of transportation to make home visits.

Willingness to work weekends, holidays, and on-call in accordance with departmental needs and rotation.

Weekend rotations are once every 6 weeks.

Description :

Providing clinical services to patients from birth to over 100 years in their homes.

Assessing patient conditions and implementing care plans.

Providing ongoing education and support to patients and their families.

Performing admission processes and case management responsibilities in accordance with policies and standards of practice.

Utilizing thorough and timely electronic documentation and interdisciplinary communication.
