At Northwest Physical Therapy, we are looking for a dynamic Full-Time or Part-Time Occupational Therapist to join our expanding Home Health team.  The Occupational Therapist's role is to act as the patient’s partner throughout the journey of restoring motion and ensuring that the client will function at their personal best in their own home. As a home health therapist, you can set your own hours, and manage your own schedule! No weekends, no holidays, no evenings, no On-Call!

Responsibilities :

• Identify and meet patients’ goals and needs through your skilled evaluation and assessment.
• Offer cost-effective treatments that help improve patient’s motion and mobility in their home.
• Develop care plans using a variety of treatment techniques and coordinate with your assistant to carry out such plans.
• Provide quality, personalized and evidence-based care and proven interventions.
• Motivate patients during treatment in order to help them function optimally.
• Document patient care services.
