Hospice Aide - Hospice House

Hospice Aides provide personal care and designated health related services in compliance with policies and procedures, applicable health care standards, and government laws and regulations.

Hours : 6am-6 : 30pm

Requirement : Active CNA in the State of IL

Responsibilities :

• Assists in the implementation of the patient Plan of Care.
• Assists patients with activities of daily living.
• Attends in-services, staff meetings, and continuing education.
• Follows verbal and written directions provided by the RN, specific to the patient assigned.
• Maintains a clean and safe environment and performs light housekeeping assignments as assigned.
• Maintains and submits documentation as required. Records observations, completed, and refused tasks.
• Observes, records, and reports significant changes in the status of a patient and / or family to supervisor.
• Participates in interdisciplinary team meetings.
• Prepares light meals if assigned in patient plan of care.
• Provides hands-on personal care to patients.
• Provides personal care and supportive services to hospice patients.
