This is a dual role in Bemidji and Blackduck area. We have a facility in both locations

JOB DUTIES

  • Manage and supervise staff.
  • Facilitate staff meetings ongoing.
  • Stay current on rules and regulations in Housing with Services, Assisted Living, waivered services and Home Care regulations. Implement procedure and / or policy changes as needed.
  • Maintain licensure as a LALD, complete ongoing training as required.
  • Follow-up with referrals, schedule and lead tours.
  • Ensure resident documentation is completed, including admission and discharge paperwork, resident progress notes and other pertinent documentation.
  • Work with the RN regarding appropriate notification / follow up of resident concerns and / or health changes.
  • Mediate challenging situations as they arise.
  • Work with funding sources regarding contracts regarding rates and services.
  • Maintain resident relations including addressing violations and other tenet concerns.
  • Assists with service delivery as necessary, including provide direct care support if needed.
  • Willing to be on-call for emergencies per notification procedures.

QUALIFICATIONS

  • Current licensure as LALD is required.
  • A minimum experience of 2 years working with adults with disabilities and / or the senior population in long term care.
  • Supervisory experience in long term care or the human services field.
  • Ability to communicate effectively orally and in writing.
  • A familiarity and contacts with the social and supportive services in the community.
  • Must pass a criminal background check per the requirements of the State of MN.
  • Knowledge of housing rules and regulations.
  • Have a positive, compassionate, and caring approach with the ability to build relationships with residents and teams.
  • Strong organizational skills.
  • A degree in Healthcare Administration, current Registered Nurse or Licensed Practical Nurse licensure, or Social Work degree are preferred.
