- Provides general patient care and basic patient education as directed by providers.
- Performs duties which indirectly support patient care such as PCMH quality measures (prevention & disease), in-house practice services (medical, dental and behavioral health), recordkeeping, routine laboratory duties, general equipment maintenance, basic radiology, immunizations, and maintaining site supply & medication inventories.
PRIMARY DUTIES AND RESPONSIBILITIES :
Schedules and coordinates paperwork for patient / provider appointments and surgeries including referrals and pre-authorizations for specialists, diagnostic testing, and medications (as needed).Assists providers with examinations, treatments, testing, and procedures.Performs patient triage in-house and by phone.Enters nursing charges and patient related information into electronic health record software, sample medication log sheets, and other assigned log sheets.Reviews patient charts, checks in patients, obtains and records all patient’s vitals, histories (allergies, medication, medical, surgical, family, social), and reason for patient’s visit.Records and reports observed symptoms, reactions, treatments, and changes in patient’s conditions.Administers medications, immunizations, and applies sterile dressings.Actively participates in in-service education programs as required by practice.Maintains records for immunizations and other abnormal lab results as required.Maintains and stocks inventory of office and / or clinical supplies, as appropriate to position.Educations patients regarding health-related issues.Coordinates labeling, delivery, and reception of laboratory specimens and results provides information and coordinates follow-up visits regarding laboratory results.Practices safety, environmental, and / or infection control methods.Develop and assist on follow up of patient care plan.Actively participates in team huddles (organizational planning and their own team.Participates in chart audit and peer review activities as assigned.Maintains clean workspaces and areas at all times.Reports any incidents, maintenance issues, unsafe conditions, and / or good catches tosupervisor.Transportation to rotate to any of the DSHC sites, as necessary.Employee knows DSHC’s 5 core values and demonstrates the specific valued behaviors on a daily basis.Performs miscellaneous job-related duties as assigned.OTHER CLINICAL DUTIES (AS ASSIGNED) :
Performs immunization coordinator duties for three sites : Monthly accounting and ordering of all children’s Idaho Immunization Program vaccines, immunization education for staff on protocols and standards, Patient Reminders, and tracking Uniform Data System (UDS) children’s immunization reporting counts.Performs Perinatal / Prenatal Management duties for Provider :
Monthly accounting of new prenatal patients, delivery / information, new baby sex birth weights, patient reminders, patient education of prenatal cycles / hospital, set appointments for practice & hospital registration, and warm handoffs to delivering physician.All preparation for Uniform Data System (UDS) prenatal delivery and birth weights reporting.Maintains assigned clinic supply ordering and inventory :
Orders supplies / medication through vendor portal for designated site timely and accurately to maintain supply / medication inventory.Performs basic radiology images complying with radiology standards of positioning and procedures.Including performance of exposure penetration levels, usage of cassette sizes, use of R / L markers, artifact, entire x-ray of examined anatomy, correct number of views, collimation, and labeling.Provides information as needed to x-ray vendor and coordinates follow-up visits regarding x-ray results.Maintains digital equipment and software.Specialized skills in :
