Requirements
A bachelor\'s degree in the field of human services is required. Examples include psychology, social work, counseling, or a related field. Conduct substance abuse screenings, assessments, and develop individualized treatment plans.Co-facilitate and / or facilitate individual, group, and family counseling sessionsProvide case management, including linking clients to community resources and services, including healthcare providers and community organizations.For court mandated clients, conduct case management and completion of all progress reports and communication with juvenile justice system.Maintain accurate and timely client records in compliance with all regulations.Collaborate with a multidisciplinary team to ensure holistic client care.Participate in professional development and training.Provide ongoing care coordination and advocacy to help individuals achieve goals and eliminate challenges to program participation (transportation, childcare, virtual supports, etc).Providing ongoing support and advocacy to help clients achieve and maintain recovery.About the Company
