LPN - Full-Time Days-Eddy Senior Care

• Eddy SeniorCare has a rewarding opportunity for a Licensed Practical Nurse (LPN) to work out of our 1938 Curry Road, Schenectady NY 12303 location!
• This is a full time position supporting both our primary clinic and our home based participants. PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare / Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team). The primary purpose of this job position is to provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders. Care is performed at our center as well as in our participant's homes. Duties include preparing participants visits in the medical clinic, drawing labs, administering vaccines & collecting specimens. In addition, this role supports the management of our participants in the community including medication delivery and dispenser set up, wound care and other treatments as ordered. The LPN may assist in the oversite of unlicensed staff, ie. home health aides. The candidate should have experience or willingness to become proficient in phlebotomy, and IV administration of fluids and medications within scope of practice. • A current license to practice as a Licensed Practical Nurse (LPN) in the State of New York
• 1 year or more of experience as an LPN
• Licensed Practical Nurses must possess valid Basic Life Support (BLS) certification or achieve BLS certification within 90 days of starting via on site education
• Valid Driver License and reliable vehicle are required Preferred :
• Experience as a LPN in a home care or skilled nursing facility
• Licensed Practical Nurses at Eddy SeniorCare must possess a passion for caring for seniors and demonstrate a respectful, caring, professional demeanor
• The LPN must have strong time management and organizational skills
• The LPN must be able to communicate effectively, verbally and written Pay Range : $24.00 - $30.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
