Social Worker LCSW - Palliative Care Inpatient - Relocation Assistance Offered

Description

This position is supporting Palliative Care Inpatient.

Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and / or palliative services to patients and their families. Works as a team member to identify problems and assist patient / family members in exploring realistic options, decision making and problem solving.
