Salary : $85,000.00 - $94,000.00 per year

Essential Duties and Responsibilities :

• Provide direct patient care during after hours, addressing both scheduled and unscheduled needs.
• Respond to patient and caregiver needs via telephone, home visits, or facility visits for assessments, symptom management, and treatment.
• Rotate on-call responsibilities on a 7 on / 7 off schedule, starting each on-call period at 4 : 30 PM and covering all hours, including entire weekends.
• Work collaboratively with a backup Registered Nurse Case Manager (RNCM), a Supportive Care Team Member, and an on-call Administrator to ensure comprehensive patient care.
• Maintain accurate and detailed patient records in accordance with regulatory standards and organizational policies.
• Teach patients, families, and caregivers proper end-of-life care techniques and measures.
• Participate in interdisciplinary team meetings to review cases and coordinate plans that align with patient care goals.
