General Responsibilities : The individual will be responsible for providing QDDP Services for adults and children with intellectual and developmental disabilities.
The QDDP assumes responsibility for the following roles to include :
Records management
Records review / audits
Conducting and approving assessments
Prescreening, admitting, and discharge planning
Intake screening and associated paperwork
Attend Intake, SIS, and Annual meetings
Overseeing the development and implementation of Individualized Service Plans (Part V Initial and Annual)
Writing reports such as Quarterly Service Reviews
Reviewing documentation on the individuals
Coordinating services with Support coordinator to include scheduling meetings
Knowledge, Skills, and Abilities :
Experience managing services for adults and children with intellectual, developmental, physical, and mental disabilities
Experienced in personnel management, HIPPA and records management, developing and implementing Individualized Service Plans, conducting, and approving assessments, prescreening, and discharge planning
Familiarity with regulations governed by DBHDS and Offices of Human Rights, Medicaid, Department of Labor, OSHA, Community Service Boards, Case Management Services, First Aid, and CPR
Effective written and oral communication skills
Strong documentation skills
Knowledge of behavioral plans
Qualifications :
A Bachelors degree plus one year of experience and supervisory skills working with the intellectually and developmentally disabled population served
Must have a valid Virginia drivers license
Must have one (1) year of working with the intellectually and developmentally disabled population
Certifications are preferred for Therapeutic Options, First Aid, and CPR. Will provide this training if necessary.
