Position Title : Case Manager I

POSITION SUMMARY

The Case Manager I plays a critical role in coordinating and facilitating comprehensive care plans for clients within the health care services sector, specifically at the MLK BH Center. This position focuses on assessing client needs, developing individualized service plans, and ensuring access to appropriate medical, social, and community resources. The Case Manager I acts as a liaison between clients, healthcare providers, and support services to promote optimal health outcomes and improve quality of life. Through continuous monitoring and follow-up, the Case Manager I ensures that clients receive timely interventions and support tailored to their unique circumstances. Ultimately, this role contributes to the overall effectiveness and efficiency of care delivery within the MLK BH Center by fostering collaboration and advocating for client-centered solutions.

LICENSES / CERTIFICATES

For AOD programs, registration or certification with a DHCS approved state level counselor certification agencies.
